Transitional Care Planning & Coordination
Vibrant Care Pharmacy can work with hospitals’ to support discharge planning; readmission reduction strategies.
We work closely with our partners to customize a pharmacy program that fits best into their benefit design — for both immediate and long-term savings, and better patient care.
We work in partnership with the hospital outpatient pharmacy to provide patient on-boarding, copay assistance program enrollment; prior authorization coordination and drug therapy and side effect management.
